30-day deadline for submitting each HOPE record to iQIES
The time-limited period — 30 days for most assessment types — within which a completed HOPE assessment must be transmitted to CMS; assessments submitted outside the window are counted as non-compliant.
The HOPE Submission Window is the 30-day period, measured from each record's target date, within which a hospice must submit and have that HOPE record accepted into iQIES to count toward Hospice Quality Reporting Program (HQRP) compliance (HOPE Implementation FAQs)[1]. HOPE carried over the same 30-day deadline that applied under HIS.
Every HOPE record, whether an Admission, an HUV, or a Discharge, has a target date, and the record must be accepted into iQIES within 30 days of that date (HOPE Implementation FAQs)[1]. Timeliness is scored against the HQRP compliance threshold: a hospice must have at least 90 percent of its required HOPE records accepted within the 30-day deadline across the reporting year (Timeliness Compliance Threshold Report fact sheet, updated January 2026, https://www.cms.gov/files/document/hospice-timeliness-compliance-report-fact-sheet-jan-2026-508c.pdf-0). The 90 percent threshold has been in effect since the FY 2020 APU determination. Admission, HUV, and discharge records are all analyzed for timeliness, so a late record of any type counts against the ratio (Getting Started with the HQRP, updated January 2026, https://www.cms.gov/files/document/getting-started-hqrp-jan-2026-pdf.pdf). CMS provides timeliness reports in iQIES so hospices can monitor their acceptance rate against the threshold before the compliance determination.
The 30-day window is where good clinical documentation quietly turns into a penalty if submission lags. A hospice can complete every HUV on time and still fail HQRP if records sit un-transmitted past day 30, because compliance is scored on acceptance into iQIES, not on assessment completion. The 90 percent threshold permits a 10 percent margin across the whole year, and with HOPE generating up to four records per patient, total record volume is higher than under HIS, so the absolute number of late records the threshold allows in absolute terms is small. The iQIES timeliness report is available on a monthly basis, enabling hospices to identify compliance gaps before the annual determination rather than discovering non-compliance after the APU adjustment is applied.
30 days from the record's target date; the record must be accepted into iQIES within that window to count as timely (HOPE Implementation FAQs)[1].
At least 90 percent of required HOPE records must meet the 30-day deadline across the reporting year to satisfy the HQRP timeliness threshold (Timeliness Compliance Threshold Report fact sheet, January 2026, https://www.cms.gov/files/document/hospice-timeliness-compliance-report-fact-sheet-jan-2026-508c.pdf-0).
Submission and acceptance into iQIES, not completion, determines timeliness, so a completed but un-transmitted record can still be scored late (Getting Started with the HQRP, January 2026, https://www.cms.gov/files/document/getting-started-hqrp-jan-2026-pdf.pdf).
No; HOPE kept the same 30-day submission deadline that applied under HIS (HOPE Implementation FAQs)[1].
